What to Expect the First 2 Weeks After Lipo 360

The first fourteen days decide a lot about your final contour. Here is exactly what happens, day by day, and what your job is at each stage.

Lipo 360 treats the whole midsection as one unit — upper and lower abdomen, both flanks (love handles), and the mid-to-lower back — so the waist is shaped in the round instead of flattened from the front only. Because it is circumferential, the recovery feels different from a single-area case. More surface area was treated, so there is more swelling, more drainage, and more tightness.

Nearly all of my Lipo 360 patients are done awake, under tumescent local anesthesia. That changes the first 48 hours considerably: no general anesthetic to sleep off, no airway soreness, far less nausea, and you walk out of the office on your own. What follows is the timeline I give patients in pre-op.

Day of surgery

You will be sore, tight, and leaking. All three are normal.

Days 1–3: the hardest stretch

Peak discomfort is usually day 2 or 3, not day 1. Patients describe it as a deep, all-over muscle soreness — like the worst core workout of your life — rather than sharp pain. Sitting up from lying flat and getting out of a car are the hardest movements. Roll to your side and push up with your arms.

Call me right away for: fever above 101.5°F, spreading redness or warmth at an incision, foul-smelling drainage, calf pain or swelling on one side, chest pain, or shortness of breath. Those are not part of normal recovery.

Days 4–7: turning the corner

Most patients feel meaningfully better around day 4 or 5. Desk work is realistic in this window — many of my patients go back at day 3–5, and I ask them to keep it to short days at first. Physical or on-your-feet jobs need 10–14 days.

Days 8–14: firm, lumpy, and normal

Weeks two through six are the fibrosis phase. The tissue that was treated lays down scar collagen as it heals, and it feels hard, ridged, or lumpy — often unevenly, one flank firmer than the other. Almost every patient interprets this as a bad result. It is not. It is a healing stage, and it softens with time, massage, and compression.

What your result looks like at 2 weeks

Honestly? You will see the waistline appearing, especially in the flanks and back, and you will still be swollen enough that you doubt it. Rough guide:

Do not judge your result at two weeks. Take photos at the same angles and lighting weekly instead — the week-to-week change is what tells the truth.

The five things that actually change your outcome

  1. Wear the garment as prescribed, all of it, not most of it.
  2. Get your lymphatic massage series. Do not skip sessions because you feel fine.
  3. Walk daily from day one; wait on real training until cleared.
  4. Hydrate, keep protein up, keep sodium down while swollen.
  5. Come to your follow-ups so I can catch a seroma or an area of firmness early, while it is easy to treat.

Lipo 360 removes fat permanently, but the fat cells left behind can still enlarge. A stable weight is what protects the contour long term. If weight is part of the picture, we often sequence medical weight loss before contouring rather than the reverse.

Frequently asked questions

How long does Lipo 360 recovery take?

Most patients return to desk work in 3 to 5 days and feel largely normal by 2 weeks. Compression is worn about 6 weeks, full workouts resume at 4 to 6 weeks with clearance, and the final contour settles between 3 and 6 months.

How long do you drain after Lipo 360?

Usually 24 to 48 hours, occasionally up to 72. The incisions are left open on purpose so the tumescent fluid drains out instead of staying in your tissue, so heavy pink-orange drainage in the first two days is a good sign, not a problem.

How long do I have to wear the compression garment?

About 23 hours a day for the first two weeks, off only to shower, then a lighter garment through roughly week 6. Garment compliance is one of the few parts of the result that is entirely in your control.

Is it normal to be hard and lumpy after Lipo 360?

Yes. Firmness and lumpiness peak between about week 2 and week 6 as healing collagen forms, and it is often uneven side to side. It softens with time, compression and lymphatic massage. Tell me about any area that is firm, tender and growing, so I can rule out a seroma.

When can I go back to the gym after Lipo 360?

Walking starts the day of surgery. Light cardio around day 10 to 14. Lifting, core work and high-impact training wait until 4 to 6 weeks and until I clear you — training through swelling sets your recovery back rather than speeding it up.

Why do I look bigger after Lipo 360?

Swelling. Fluid retention in the treated tissue can briefly make your waist measure larger than it did before surgery, usually peaking in the first one to two weeks. It resolves over the following weeks, faster with compression, walking and lymphatic massage.
